Transition Services for Students with IEPs

The Individuals with Disabilities Education Act (IDEA) serves to ensure that students with disabilities are guaranteed a free, appropriate public education (FAPE). Once a student approaches the age of 16 within that IEP year (the annual Individualized Education Plan), post-secondary transition planning must be implemented. By definition transition planning simply means preparing for the next step in our future.

In accordance with federal law, the transition IEP must include the following:

Present level of academic performance as well as skills and supports needed for post-secondary life
Appropriate measurable postsecondary goals with action steps leading to successful completion
Modifications and accommodations necessary for each student
A current action plan to be incorporated addressing employment preparation/training, education preparation, and independent living skills/community access
