Transition Services for Students with IEPs
In accordance with federal law, the transition IEP must include the following:
- Present level of academic performance as well as skills and supports needed for post-secondary life
- Appropriate measurable postsecondary goals with action steps leading to successful completion
- Modifications and accommodations necessary for each student
- A current action plan to be incorporated addressing employment preparation/training, education preparation, and independent living skills/community access
